You need to go to the court for that.
Why, if you all agree, what will it give you?
Try legalzoom.com. Or you can just go to your local courthouse and ask for one.
It may not be that simple. What does he want in exchange?
I’d strongly suggest you speak with a family lawyer before going ahead with this. If you can’t afford one, you can contact Legal Aid.
